If you were an LCMC patient portal account holder who access their LCMC patient portal account between January 1, 2019, and November 30, 2022, you may be entitled to receive a $15.00 cash payment and/or the Privacy Shield product.

If you wish to receive a cash payment, you must submit a Claim Form by November 25, 2025.

Cash Payment: Class Members are eligible to claim a cash payment of $15.00

Privacy Shield Services: The settlement also automatically provides all Settlement Class Members with access to Privacy Shield Pro Services for a period of one (1) year after the Settlement is final.

You may get a payment if you timely fill out and submit a claim form, if the Settlement is approved, and if you are found to be eligible.

To start your online claim form, you can find your Unique ID & PIN printed on the Notice that was sent to you. If you misplaced your notice, please contact the Settlement Administrator at info@LCMCDataSettlement.com or 844-496-0605. Please request your Unique ID & PIN by providing your full name and mailing address.

The deadline to submit a Claim Form online is November 25, 2025. If you are mailing your Claim Form, it must be mailed with a postmark date no later than November 25, 2025.
